Families of Kindergartners, we need your help!
Please take a few minutes to complete an important online survey about your child’s experiences before they started Kindergarten. Your feedback will help us improve the programs and services for your child and all children in our community.
The survey is called the CHEQ and is part of a statewide effort supported by the New Mexico Public Education Department and the Early Childhood Education and Care Department. It will be available between October 1 and December 1, 2024.
If you have any questions, feel free to reach out to your child’s lead teacher or principal, or contact the FMS District Test Coordinator, Linda Kerr, at lkerr@fms.k12.nm.us or 505-599-8820, ext. 1581.
How to complete the survey:
You will need your child’s State Student ID, which can be found in the PowerSchool parent portal. The ID is a 9-digit number listed below your child’s name. Please enter this number carefully.
You can start the survey by clicking here.
The survey is available in 8 languages: English, Spanish, Diné, Tagalog, Arabic, Khmer, Traditional Chinese, and Vietnamese.
Thank you for taking the time to share your thoughts with us! Your input is valuable and will make a difference in shaping our programs.
Other helpful tips:
Participation in this survey is voluntary but strongly encouraged.

You can complete the survey at your child’s school, at home, or wherever is most convenient.
The survey takes approximately 15-20 minutes.
Only one adult per child should complete the survey.
